When Sam looks at Dean he doesn't see his brother any more. Instead he sees a man who loved him enough to go to Hell for him. He sees the man who went to Hell, who had to dig himself out of his own grave. He sees a corpse sometimes and then he just has to reach other and touch Dean because there is nothing corpse like about his brother's firm, youthful body.

When Dean looks at Sammy he no longer sees through the eyes of a brother. He sees though the eyes of a man who has forgotten something that he never wants to remember but feels compelled to seek out. He looks through the eyes of a man who has died and been given a second - no third, no forth chance - he sees through the eyes of a man who no longer exits.

When Dean looks at Sammy he no longer sees a brother; but a cause worth dying for, something precious enough to make Hell worth while. But he sees this because he has to. Because if he knew the truth - that Sam never kept his promises, that Sam is now more tainted by demon blood than ever - then he would just climb voluntarily back into hell and allow himself to be ripped to shreds by guilt and pain and fear.

When Sam looks at Dean he sees the brother he lied to. SO maybe it's easier to just see the man who loves him.

Maybe this is why neither one of them is disturbed as their mouths find each other, their hands caress each other, and their bodies fit together in an act of passion usually so forbidden that even the Devil would turn his eyes from the sight.
